Responsibilities
- Discharge planning
- Emergency Department (ED) experience
- Palliative Care knowledge and Goals-of-Care (GOC) conversations
- Behavioral Health experience with substance use assessments
Requirements
- Minimum 1 year of experience as a Social Worker, preferably in an acute care setting
- Active Colorado (CO) Social Worker license
Schedule
- 40 hours per week
- 8-hour day shifts
