Travel Skilled Nursing Facility (SNF) Physical Therapy Assistant - $1,713 per week
About the role
Jackson Therapy Partners is seeking a travel Skilled Nursing Facility (SNF) Physical Therapy Assistant for a 13-week assignment starting July 27, 2026, in Santa Rosa, California.
Responsibilities
- Work with patients recovering from injuries or illness, helping them move better, feel better, and stay active under the guidance of a licensed Physical Therapist.
- Support at least one weekend day.
- Use Net Health and PCC software.
- Manage a caseload of 9 to 12/13 patients, including group treatment when applicable.
- Work with a predominantly short-term care population, around 80%.
Requirements
- Associate degree in Physical Therapy from an accredited educational program.
- Active State License to start the assignment.
- BLS Certification from AHA or ARC.
- Experience with Net Health and PCC.

Pay & Schedule
The pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
Schedule
5x8 schedule with a weekend requirement to support at least one weekend day.
