Travel Skilled Nursing Facility (SNF) Physical Therapist - $2,667 per week
About the role
Ardor Health Solutions is seeking a travel Physical Therapist for a Skilled Nursing Facility / Post-Acute Rehab contract in Unity, New Hampshire. This 14-week assignment (also noted as 13 weeks) runs from October 26, 2026 through January 24, 2027, with a 40-hour weekly guarantee and 8-hour day shifts. The role serves adult and geriatric patients, focusing on post-acute rehabilitation to improve mobility, strength, balance, and functional independence. A $2,000 quick-start bonus is available for qualifying candidates.
Responsibilities
- Perform physical therapy evaluations and reassessments
- Develop individualized treatment plans based on patient needs and functional goals
- Provide therapeutic exercise, gait training, balance activities, transfer training, and mobility interventions
- Help patients improve strength, endurance, safety, and independence
- Educate patients and caregivers on mobility strategies and home exercise programs
- Collaborate with nursing staff, occupational therapy, speech-language pathology, and the interdisciplinary rehab team
- Complete accurate and timely documentation
Requirements
- New Hampshire Physical Therapist license (required)
- BLS certification (required)
- SNF or post-acute care experience (preferred)
