Travel Skilled Nursing Facility (SNF) Physical Therapist - $2,566 per week

Ardor Health Solutions is seeking a Physical Therapist for a 13-week travel assignment at a Skilled Nursing Facility in Lake Worth, Florida. This role focuses on evaluating and treating adult and geriatric patients recovering from hospitalization, surgery, illness, or injury to improve strength, balance, mobility, safety, and functional independence.

About the Role

• Skilled Nursing Facility (SNF) setting
• 13-week travel assignment
• Full-time schedule (40 hours per week, 8-hour day shifts)
• Adult and geriatric rehab population
• Short-term rehabilitation and longer-term patient needs
• Focus on gait, balance, strengthening, and mobility training
• Collaborative interdisciplinary rehab team

Responsibilities

• Complete PT evaluations and reassessments
• Develop individualized plans of care
• Provide gait, balance, strengthening, transfer, and mobility training
• Help patients improve functional mobility and independence
• Provide fall-prevention and safety education
• Educate patients and caregivers
• Collaborate with PTA, OT, COTA, SLP, nursing, and other care team members
• Assist with discharge planning and recommendations
• Complete timely clinical documentation

Requirements

• Graduate of an accredited Physical Therapy program
• Current Florida PT license or ability to obtain prior to start
• Current BLS certification
• Strong evaluation and treatment skills
• Strong documentation and communication skills
• Comfortable working with adult and geriatric patients
• SNF or geriatric rehab experience preferred
