About the role
Medlivo Therapy is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week contract starting August 17, 2026.
Responsibilities
- Provide physical therapy services to patients in a Skilled Nursing Facility setting.
- Develop and implement individualized treatment plans based on patient needs.
- Assess patient progress and modify treatment plans as necessary.
- Document patient care activities and progress in accordance with facility policies and regulations.
Requirements
- Active Physical Therapist License.
- Current BLS/BCLS Certification.
Qualifications
- Experience in providing physical therapy services in a Skilled Nursing Facility setting preferred.
- Strong clinical skills and knowledge of physical therapy techniques.
- Excellent communication and interpersonal skills.
Skills
- Skilled in assessing and treating physical therapy needs.
- Proficient in documenting patient care activities.
- Ability to work independently and as part of a team.
Pay
Weekly Gross: $2,560.00
Schedule
40 hours per week, 8-hour shift.
