About the role

Pioneer Healthcare Services is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week assignment in Encinitas, California. This on-site position offers day shifts of 8 hours per week, totaling 40 hours per week.

Responsibilities

  • Assess patients' physical abilities and develop personalized treatment plans
  • Provide therapy to improve mobility, strength, and functional independence
  • Collaborate with healthcare teams to ensure comprehensive patient care
  • Educate patients and families on exercise programs and injury prevention
  • Monitor and document patient progress and adjust treatment plans as needed
  • Implement interventions in various settings
  • Participate in discharge planning and coordinate follow-up care for continued rehabilitation

Requirements

  • Doctorate in Physical Therapy
  • Active California Physical Therapist (PT) license
  • Current BLS/CPR Certification

Pay

$2,454.77 - $2,530.69 per week

Schedule

  • 13-week assignment
  • Day shift, 8 hours per day
