Travel Skilled Nursing Facility (SNF) Physical Therapist - $2,475 per week
About the Role
This travel Physical Therapist position is for a 13-week contract in a Skilled Nursing/Rehab facility in Peabody, MA (North Shore). The role involves working primarily with adult and geriatric rehabilitation patients to regain strength, mobility, balance, and functional independence following hospitalization, surgery, illness, or injury. The schedule is 40 hours per week, 8-hour day shifts.
Responsibilities
- PT evaluations and reassessments
- Individualized plans of care
- Therapeutic exercise and strengthening
- Gait and balance training
- Transfer and functional mobility training
- Fall prevention and safety education
- Assistive device training
- Patient and caregiver education
- Collaboration with OT, SLP, nursing and the interdisciplinary rehab team
- Discharge planning
