Travel Skilled Nursing Facility (SNF) Physical Therapist - $2,349 per week

About the role

A physical therapist specializing in skilled nursing facility (SNF) care is needed for a 13-week assignment starting August 25, 2026.

Responsibilities

• Provide physical therapy services to patients in SNF settings
• Develop and implement individualized treatment plans
• Evaluate patient progress and adjust treatment plans accordingly
• Collaborate with interdisciplinary teams to ensure patient care coordination
• Document patient care activities and progress in medical records

Requirements

• Current license to practice physical therapy in Montana
• Experience in SNF or acute care setting preferred
• Knowledge of Medicare and Medicaid regulations
• Excellent communication and interpersonal skills

Qualifications

• Master's degree in physical therapy from an accredited program
• Passing score on the National Physical Therapy Examination (NPTE)
• ACSM Certified Personal Trainer (CPT) preferred

• Ability to work independently and as part of a team
• Proficient in computer software for documentation and record-keeping
