Travel Skilled Nursing Facility (SNF) Physical Therapist - $2,210 per week

About the role

CoreMedical Group is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a 13-week assignment starting July 12, 2026, in Tulsa, Oklahoma.

Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility setting.
• Develop and implement individualized treatment plans based on patient needs.
• Assess patient progress and modify treatment plans accordingly.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Document patient care activities and progress in medical records.

Requirements

• Current licensure to practice as a Physical Therapist in the state of Oklahoma.
• At least one year of clinical experience in a skilled nursing facility setting preferred.
• Knowledge of Medicare and Medicaid regulations.
• Excellent communication and interpersonal skills.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Passing score on the National Physical Therapy Examination (NPTE).

Skills

• Strong clinical skills and knowledge of physical therapy techniques.
• Effective problem-solving and decision-making abilities.
• Ability to work independently and as part of a team.
• Proficiency in computer software for documentation and record-keeping.

Pay
