Travel Skilled Nursing Facility (SNF) Physical Therapist - $2,166 per week
Ardor Health Solutions is seeking a travel Physical Therapist for a Skilled Nursing Facility contract in Chicago, Illinois. This post-acute rehab opportunity offers the chance to work with adult and geriatric patients while helping them improve mobility, strength, balance, transfers, and overall functional independence.

- Full-Time Contract: 40 hours per week, 8-hour day shifts
- Start Date: 9/7/26, End Date: 12/5/26 (13 weeks)

Responsibilities
- Perform comprehensive physical therapy evaluations and reassessments.
- Develop individualized treatment plans based on patient goals and physician orders.
- Treat patients with orthopedic, neurological, post-surgical, and medically complex conditions.
- Provide gait training, balance training, therapeutic exercise, transfer training, and functional mobility interventions.
- Educate patients and caregivers on safety strategies and home exercise programs.
- Collaborate with nursing staff, OT, SLP, and the interdisciplinary rehab team.
- Complete timely documentation and progress reporting.
Requirements
- Illinois PT License Required
- BLS Required
- SNF or post-acute experience preferred
