Travel Skilled Nursing Facility (SNF) Occupational Therapist - $2,526 per week
Ardor Health Solutions is seeking an Occupational Therapist for a travel assignment at a Skilled Nursing Facility in Stroudsburg, Pennsylvania. This role focuses on helping adult and geriatric patients regain independence with daily activities following hospitalization, surgery, illness, or injury.
About the Role
- 13-week travel assignment at a Skilled Nursing Facility (SNF)
- Full-time schedule (40 hours per week, 8-hour day shifts)
- Adult and geriatric rehab population
- Focus on ADLs and functional independence
- Interdisciplinary rehab team collaboration
- Mix of short-term rehabilitation and longer-term patient needs
- Start date: 08/31/2026
Responsibilities
- Complete OT evaluations and reassessments
- Develop individualized plans of care
- Address ADLs, strength, endurance, mobility, cognition, and safety
- Provide therapeutic activities and functional training
- Educate patients and caregivers
- Make adaptive equipment recommendations
- Collaborate with PT, PTA, SLP, nursing, and the interdisciplinary team
- Assist with discharge planning and recommendations
Requirements
- Graduate of an accredited Occupational Therapy program
- Current Pennsylvania OT license or ability to obtain prior to start
- Current BLS certification
- Strong evaluation, treatment, and documentation skills
- Comfortable working with adult and geriatric patients
- SNF or geriatric rehab experience preferred
