Job Requirements
- Current nursing license with Nursys verification within 30 days prior to start
- Completion of FACIS Questionnaire if any disciplinary actions
- BLS certification (AHA)
- Completion of Ascension Core Competencies with minimum 80% passing score
- Completion of Ascension EKG Exam with minimum 80% passing score
- Completion of Ascension Skills Checklist within one year of start, updated annually for specialty
- Documentation of required immunizations and health screenings including MMR, Hepatitis B, Varicella, Tdap, Influenza vaccine (optional), 2 Step PPD or IGRA, Positive PPD with negative chest X-ray and annual TB questionnaire, Health Statement of good health
- Fit Test required prior to start or onsite, updated annually
- Completion of Ascension Onboarding Requirements Education Attestation and Nursing Contract Worker Onboarding
- Completion of Ascension Nova StatStrip pre-hire modules
- MOAB Moderate Risk De-Escalation Training
- Cerner test results and Training Checklist with signed attestation
- Ascension Badge Photo (forward facing headshot)
- Two professional references prior to first assignment
- Criminal Background Check including Social Security Trace, OIG Search, National Sex Offender, and others within 90 days of first assignment
- EPLS check within 90 days of start
- Education verification relevant to clinical work within 90 days of start
- Employment verification for past seven years
- Driver's License/State ID must be current and match address requirements

- Ascension Resume required for submission
- Signed Assignment Agreement required for onboarding
- Ascension Color Vision test completed prior to start and updated every 3 years
- NIH Certification updated yearly or every 2 years depending on group
- Ascension Unit Specific Checklist optional for onboarding but needs to be uploaded by unit leader within a week
- Hiring Manager Evaluation post-assignment (optional)
- Ascension Name Change Document (optional)
