Job Details

Provides each Elder with routine daily nursing care and other desired services in accordance with the Elder’s assessment, choices and care plan.

Works closely with the RN Manager to manage the day-to-day functions.

Facility is a total of 160 beds.

Resident to Nurse ratios will be discussed during the interview.

Previous Epic experience within the last 12 months in a LTC setting required.

Job Requirements

• Must have BLS and ACLS certifications.
• Previous Epic experience within the last 12 months in a LTC setting required.
• Primary Source Verification of professional license required within 30 days of start (for RNs use BON & NURSYS).
• Must have valid professional license/certification as required for the position.
• Completion of GRH Skills Checklist within 12 months required.
• Influenza Vaccine or Facility declination accepted, due by 12/01 annually (optional).
• Cover Letter or profile submission required.
• I-9 Attestation required.
• Color Vision test proof within 1 year.
• Evergreen Link acknowledgment must be completed within 48 hours of offer acceptance.

Schedule Information

• Variable shifts with most shifts being on Evenings (1430 - 2300) or Nights (2245 - 0645).
