Travel PT SNF/ALF - $2,320 per week

Critical Connection, inc is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a travel job in Melbourne, Florida. Start date: 10/05/2026. Duration: 13 weeks. 40 hours per week, 8‑hour day shifts.

Responsibilities

• Evaluate and treat patients recovering from illness, injury, or surgery
• Develop and update individualized plans of care (POC)
• Provide 1:1 skilled therapy to primarily Medicare patients
• Collaborate with nursing, OT, SLP, and rehab leadership
• Document evaluations, progress notes, and discharge summaries
• Educate patients and families on mobility, safety, and recovery

Requirements

• Doctorate or Master’s Degree in Physical Therapy
• Active (or pending) state PT license

Qualifications

• SNF experience preferred, not required
• Strong clinical reasoning and communication skills
• New Grad PTs welcome with mentorship available

Pay

Competitive weekly pay based on 8‑hour shifts and 40.0 hours per week (subject to confirmation) with tax‑free stipend amount to be determined.
