Travel PT SNF/ALF - $2,124 per week

Critical Connection, inc is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a travel job in West Chester, Pennsylvania. The opportunity is with a quality-driven SNF focused on restoring mobility, independence, and quality of life for short-term rehab and long-term care patients. Ideal for clinicians who value 1:1 patient care, interdisciplinary collaboration, and meaningful functional outcomes. New Grad PTs welcome with mentorship available.

Responsibilities

• Evaluate and treat patients recovering from illness, injury, or surgery
• Develop and update individualized plans of care (POC)
• Provide 1:1 skilled therapy to primarily Medicare patients
• Collaborate with nursing, OT, SLP, and rehab leadership
• Document evaluations, progress notes, and discharge summaries
• Educate patients and families on mobility, safety, and recovery

Qualifications

• Doctorate or Master’s Degree in Physical Therapy
• Active (or pending) state PT license
• SNF experience preferred, not required
• Strong clinical reasoning and communication skills

Pay

Pay package is based on 8‑hour shifts and 40.0 hours per week (subject to confirmation) with a tax‑free stipend amount to be determined.

Start date: 10/26/2026. Duration: 13 weeks. 40 hours per week. Shift: 8 hours, days.
