About the role
Assured Nursing is seeking a travel Home Health Physical Therapist for a 13-week assignment starting August 24, 2026, in Petaluma, California.
Responsibilities
- Provide physical therapy services to patients in their home environment
- Develop and implement individualized treatment plans
- Evaluate patient progress and adjust treatment plans as necessary
- Document all treatments and patient progress in accordance with regulatory requirements
Requirements
- Current state licensure as a Physical Therapist
- At least one year of recent clinical experience in Home Health Physical Therapy
- Bachelor's degree in Physical Therapy from an accredited program
- Passing score on the National Physical Therapy Examination (NPTE)
Qualifications
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
Skills
- Proficiency in using electronic health records (EHR) systems
- Knowledge of Medicare and Medicaid regulations

Pay
The pay package is based on 9 hour shifts and 40.0 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
