About the Role

American Traveler is seeking a Home Health Physical Therapist with at least one year of home health experience to support the transition to HCHB PointCare and serve as a preceptor during go-live implementation across Oregon locations.

Assignment Overview

• Shift: Days, 5x8 hours, 8:00 AM to 4:30 PM, Monday through Friday
• Hours: 40 hrs/week
• Start Date: October 12, 2026
• Length: 6 weeks
• Openings: 2

Details

• Home Health setting supporting Providence at Home with Compassus locations in the Mid-Willamette Valley
• Six-week contract combining HCHB PointCare training with on-site go-live and preceptor support
• First three weeks include HCHB PointCare training with in-person sessions Tuesday through Thursday and virtual sessions Monday and Friday
• Final three weeks focus on on-site go-live support and precepting current travelers and staff
• Minimal bedside patient care expected, aside from occasional ride-along support during implementation
• Uses HCHB PointCare/mobile app for charting

Requirements

• Minimum one year of Home Health experience required
• Oregon license required if applicable; license pending is acceptable
• HCHB PointCare/mobile app experience preferred
• Must be tech savvy, reliable, personable, and comfortable supporting and training others

Additional Information
