MedPro Healthcare Allied Staffing is seeking a travel Home Health Physical Therapist for an assignment in Medford, Oregon.

Responsibilities

The Physical Therapist (PT) is responsible for:

• Assessing referred patients requiring rehabilitation services, including evaluating their medical condition, functional capabilities, limitations, and restrictions.
• Establishing and administering a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of a physician.
• Setting realistic and achievable goals for patients, documenting and recording their condition, and educating patients and families on appropriate physical therapy methods.
• Developing a plan of care for each patient.
• Providing skilled physical therapy services and interventions in accordance with physician orders.
• Assessing patient needs, planning for, evaluating, and modifying care to meet goals of physical therapy interventions.
• Collaborating with all disciplines to plan and evaluate team goals for each patient.

Requirements

• Doctorate degree in Physical Therapy OR a Bachelor’s degree in Physical Therapy and passing the NPTE.
• Graduation from an accredited school.
• Minimum one year’s experience as a Physical Therapist.
• Current state license as a Registered Physical Therapist (PT) or proactively in the process of obtaining licensure.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Additional requirements may be determined by the client facility.
