Travel PT Home Health - $2,664 per week

MedPro Healthcare Allied Staffing is seeking a travel Home Health Physical Therapist for an assignment in Portland, Oregon. Assignment starts 08/31/2026, lasts 13 weeks, and requires 40 hours per week on 12-hour shifts.

Responsibilities

• Assess referred patients requiring rehabilitation services, including evaluating medical condition, functional capabilities, limitations, and restrictions.
• Establish and administer a treatment program with specific goals determined by the patient’s capacity and tolerance under physician direction.
• Set realistic and achievable goals for patients, document and record patient conditions, and educate patients and families on physical therapy methods.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services and interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate, and modify care to meet goals of physical therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

Requirements

• Doctorate degree in Physical Therapy OR Bachelor’s degree in Physical Therapy and passed the NPTE.
• Graduated from an accredited school.
• Minimum one year’s experience as a Physical Therapist.
• Current state license as a Registered Physical Therapist (PT) or actively in the application process for licensure.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Additional requirements may be determined by the client facility.
