About the Role
We are seeking an experienced Home Health Physical Therapist to join our team in West Oregon and surrounding areas. The ideal candidate will provide direct patient care in home settings, ensuring optimal rehabilitation and recovery for clients.
Responsibilities
- Deliver one-on-one patient care in home environments.
- Create and implement tailored treatment plans.
- Perform evaluations and assessments to track patient progress.
- Educate patients and families on recovery strategies and home exercises.
- Utilize OASIS documentation for patient reporting and management.
- Collaborate with interdisciplinary teams to enhance patient care.
Requirements
- Minimum of 1 year of skilled home health experience.
- Active Oregon or Washington Physical Therapist license required.
- BLS certification is required.
- Experience with OASIS documentation is highly preferred.
- Reliable transportation is required.
Schedule
Day Shift: 5x8-Hour (08:00 - 16:30)
