Description

American Traveler is seeking a licensed Physical Therapist for a Home Health assignment requiring HCHB (Pointcare) experience and OASIS documentation proficiency in the Montgomery County and lower Frederick County area.

Details

• Home health setting covering Montgomery County and lower Frederick County
• Schedule is Monday through Friday, day shifts (8:00 AM - 5:00 PM)
• No weekend or holiday shifts required
• 5x8-hour shifts per week
• 13-week contract
• Position is covering for PTO leave
• This is a field-based home health role requiring travel throughout the service area

Requirements

• Active PT license required if applicable
• Experience with HCHB (Pointcare) EMR required
• Experience completing OASIS documentation required
• Two professional references required for consideration
• Valid driver's license and Social Security card required for consideration
