Travel PT Home Health - $2,298 per week

Periodic office meetings. Must have reliable transportation current drivers license and agency-required liability insurance.' Average # of patients/day: 5-6 Points per day/week: How are points broken down 30 points per week. Each type of visit and activity (documentation travel etc.) hold a different point value. Contractor will be assigned between 5-10 patients per day that equate to roughly 6-8 points. Guaranteed Hours: No if there is a situation where the contractor is unable to meet their financial needs due to low patient census please reach out to the AM to discuss floating the contractor to a nearby clinic to pick up hours. Types of Cases: Start of Care Follow Ups Years of experience REQ: Minimum 1 full year Adult Home Health Experience First-timers accepted: (Y/N) No must have recent Home Health experience Weekend REQ: Varies per location and profession please have traveler discuss during the interview. Certs REQ: CPR BLS Is on-call REQ (Y/N) What is the req Varies per job/location please discuss with manager during interview. Territory/County covering: Must be willing to travel up to 75 miles from home clinic. Mileage reimbursed IRS Standard rate. Traveler is compensated from patient home to patient home.
