Job Summary

The Nurse Practitioner (NP) at Upstate Family Health Center provides health services on an outpatient basis to patients. The NP works collaboratively with the Physician to manage health care needs, provides continuity of care, educates and counsels patients, and coordinates services.

Essential Roles and Responsibilities

  • Assess health status through health and medical history
  • Provide health care services including evaluation, diagnosis, management, and follow-up
  • Work with patients to ensure understanding and compliance with therapeutic regimens
  • Provide health promotion, education, and maintenance services
  • Maintain clinical records according to protocols
  • Maintain clinical competence through continuing education and certification
  • Participate in program planning and policy development
  • Practice safety, environmental, and infection control methods
  • Attend department meetings and training programs
  • Comply with regulatory and agency regulations
  • Establish a positive work environment
  • Participate in team activities

Education and Experience Requirements

  • Family Nurse Practitioner License (FNP) in the State of New York
  • 3+ years of experience
  • Leadership, communication, education, and counseling skills
  • Proficiency in communication technologies
  • Organized with ability to keep accurate records
  • Knowledge of local health care and social welfare resources
  • Working knowledge of Patient-Centered Medical Home (PCMH)
