Responsibilities

• Deliver comprehensive home-based nursing assessments, interventions, and patient education.
• Perform complete medication reconciliation at every visit.
• Develop, implement, and update individualized care plans.
• Monitor patient progress and communicate changes to physicians and the care team.
• Schedule and coordinate visits across multiple counties.
• Provide high-need coverage in designated areas.
• Adapt to census changes, referral volume, and staffing needs.
• Complete SOC, ROC, Recertification, Follow-Up, and Discharge assessments.
• Submit accurate OASIS documentation within 24 hours.
• Maintain infection prevention, patient safety, and risk-management standards.
• Participate in quality improvement and audit activities.
• Collaborate with physicians, therapists, social workers, and office staff.
• Communicate patient needs and care plan changes promptly.
• Attend weekly case conferences and participate in care coordination.
• Identify and address risks such as falls, medication issues, and disease-management needs.
• Provide education to reduce avoidable hospitalizations.
• Support agency quality metrics and patient satisfaction goals.
• Meet productivity expectations (30 points weekly).
• Respond to urgent staffing and patient-care needs across the service area.
• Demonstrate flexibility with assignments based on organizational needs.

Qualifications

• Active, unrestricted RN license.
• 1–2 years of home health experience.
• HCHB experience required.
• Valid driver’s license & reliable transportation.
• Current BLS certification.
