Job Summary

A Physical Therapist (PT) specializing in home health services is sought for a position based in Rock Hill, South Carolina. The role involves providing home health care to adult patients with chronic diseases, wound care, post-stroke recovery, and post-rehabilitation needs. Candidates are required to bring their license on the first day of work.

Job Details

  • Location: Rock Hill, South Carolina

  • Schedule: 8-hour shifts, with a weekend rotation not typically including weekends unless specified as a weekend-only position (Friday/Saturday/Sunday)

Required Qualifications

  • Certification as a Basic Life Support (BLS) provider

  • Valid Driver's License

  • Proof of Insurance

  • State license

  • A minimum of 1 year of home health experience

Preferred Qualifications

  • Experience with above patient types

  • Disease teaching

  • Chronic disease management

  • Gait speed

  • Gait belt use

Additional Information

  • Use of Samsung Note 9 phones, laptop, and other equipment provided

  • Orientation consists of 5 days of facility and unit training

  • A home health branch servicing York and Lancaster counties, with potential to float into Union or Mecklenburg counties but not likely

  • Support within the department is provided by departmental leadership

  • Support and technology: Homecare Homebase EMR, departmental leadership, and necessary equipment supplied

  • This is a weekend-only position; travel up to 100 miles may be required depending on caseload

  • RTO Requests: Require manager approval

  • Scrub Color: Light Grey Therapy

  • Holidays may be worked if desired; RTO requests must be approved by management

Pay Package

The pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
