About the role
Assist in implementing personalized treatment plans, provide therapeutic exercises and modalities to improve patient mobility, monitor progress, educate patients and families, maintain accurate documentation, collaborate with healthcare teams, and support discharge planning in a Skilled Nursing Facility setting.
Qualifications
- Completion of an accredited PTA program
- Passing the National Physical Therapy Exam (NPTE)
- Licensure/certification as an IA PTA
- First Aid (BLS) certification is strongly recommended
Key Duties
- Implement treatment plans developed by physical therapists
- Provide therapeutic exercises and modalities
- Monitor and report patient progress
- Educate patients and families on exercises and safety
- Maintain detailed documentation
- Collaborate with healthcare teams for coordinated care
- Support discharge and continuity of care
Additional Details
- Work on-site in a Skilled Nursing Facility
- Day shift, up to 40 hours per week
