- Licensed Rehabilitation Therapist - Skilled Nursing Facility Location Hawaii Assignment Duration: This is a 13-week contract position. Schedule: Variable schedule - 5 days per week, including potentially one weekend day.
- Responsibilities include conducting evaluations, reassessments, and treatments, while working collaboratively with an interdisciplinary team to develop and implement individualized treatment plans.
- Responsibilities:
- Provide clinical rehabilitation services according to licensure and scope of practice.
- Develop, implement, and carry out individualized treatment plans in collaboration with the supervising therapist and physician.
- Utilize evidence-based therapeutic interventions to maximize residents' function, independence, and quality of life.
- Educate residents, families, and caregivers on treatment plans and progress.
- Complete accurate and compliant documentation of patient care activities.
- Supervise therapy assistants as applicable.
- Be available to work on Hawaii State and Federal Holidays as necessary based on facility operations.
Required Experience / Certifications / Licensure: A minimum of two (2) years of experience in a Skilled Nursing Facility or Intermediate Care Facility setting preferred. Current State of Hawaii license in good standing appropriate to the discipline. Current Basic Life Support (BLS) certification.
