About the role
Seeking a travel Physical Therapist (PT) for a contract role in a Skilled Nursing Facility (SNF) / Long-Term Care (LTC) setting.
Responsibilities
- Developing and implementing individualized care plans for patients.
- Providing skilled physical therapy interventions in accordance with physician orders.
- Assessing patient needs, evaluating progress, and modifying treatments to meet therapeutic goals.
- Collaborating with interdisciplinary teams to plan and evaluate patient care.
- Ensuring the integration of organizational values and goals into job performance.
Qualifications and Skills
- Graduation from an accredited school.
- Current state licensure or active application for licensure as a Registered Physical Therapist (PT).
- Current CPR certification.
- Experience or recent graduation in physical therapy.
- Proactive, collaborative, and committed to quality patient care.
