Travel Physical Therapist SNF / LTC - $2,291 per week

Centra Healthcare Allied is seeking a travel Physical Therapist for a contract assignment in Johnston, Rhode Island.

About the role

A great SNF / LTC facility is looking for an experienced or newly graduated Physical Therapist to join their growing team. The role involves providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation. The Physical Therapist will establish and administer a treatment program with specific goals determined by the patient's capacity and tolerance under physician direction, set realistic goals, document patient progress, and educate patients and families on physical therapy methods.

Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services and interventions in accordance with physician orders.
• Assess patient needs, plan for, evaluate, and modify care to meet goals of interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

Requirements

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist or proactively in the process of obtaining licensure.
• Current CPR certification.

Schedule

• Start Date: ASAP
• Duration: 13 weeks
• 40 hours per week

Pay

Pay package is based on 8.0-hour shifts and 40.0 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
