Travel Physical Therapist - SNF / LTC - $2,225 per week

Centra Healthcare Allied is seeking a travel Physical Therapist for a travel job in Boulder, Colorado. A great SNF/LTC is looking for an experienced or newly graduated Physical Therapist/PT to join their growing team for a contract assignment. This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan for, evaluate and modify care to meet goals of physical therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

Requirements

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist/PT or actively in the process of application for current state licensure as a Registered Physical Therapist/PT.
• Current CPR certification.

Pay

Pay package is based on 8.0‑hour shifts and 40.0 hours per week (subject to confirmation) with a tax‑free stipend amount to be determined.

Schedule

40 hours per week, 8‑hour shifts, days schedule.
