Travel Physical Therapist SNF / LTC - $2,081 per week

Centra Healthcare Solutions is seeking an experienced Physical Therapist (PT) licensed or in the process of obtaining licensure in New York to work in a Skilled Nursing Facility / Long-Term Care (SNF / LTC) setting.

About the role

This is a 12-week travel contract position starting 09/01/2026, with a schedule of 5x8 days (40 hours per week, 8-hour day shifts). The role assumes responsibility and accountability for incorporating the organization’s vision, values, mission, and critical goals into job performance.

Responsibilities

• Provide physical therapy services in a SNF / LTC environment.

Requirements

• Minimum of 1 year of recent experience as a Physical Therapist (PT).
• SNF / LTC experience required.
• Current Physical Therapist (PT) license within the state of New York or in the immediate process of obtaining licensure.

Pay

Pay package is based on 8.0-hour shifts and 40.0 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
