Responsibilities

• Conduct evaluations and develop personalized treatment plans for patients.
• Implement therapeutic interventions to aid recovery and improve patients' quality of life.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
• Document patient progress and modify treatment plans as necessary.

Requirements

• Minimum of 1 year of experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) environment.
• Valid Physical Therapist license for the state of Washington (Compact license accepted).
• BCLS certification required.
