Travel Physical Therapist SNF - $1,720 per week

Cross Country Allied is seeking a travel Physical Therapist for a skilled nursing facility (SNF) setting in Hilo, Hawaii. This role involves providing rehabilitation services including evaluations, treatments, and development of individualized care plans, ensuring compliance with state and federal regulations. The position requires working primarily days, with potential flexibility including one weekend day, and may involve floating to other units or facilities as needed.

Responsibilities

• Provide evidence-based therapeutic interventions to maximize patient function and quality of life
• Supervise therapy assistants when applicable
• Perform evaluations, reassessments, treatments, and develop individualized treatment plans
• Adhere to all facility operational policies

Schedule

• Generally scheduled for five days per week, with one weekend day
• Shift hours: 8:00 AM to 4:00 PM
• Minimum of 40 hours per week
• Duration: 13 weeks
• Start Date: 09/14/2026
• Candidates must arrive the Thursday before the start date to attend employee health appointments scheduled for Friday

Requirements

• Current State of Hawaii license in good standing appropriate to the discipline (must be active by the start of the assignment)
• Basic Life Support (BLS) certification
• Minimum two (2) years of experience in the SNF/ICF setting preferred
• Must possess an active Hawaii license by the start of the assignment
