Job Summary
A Physical Therapist specializing in Skilled Nursing Facilities (SNF) in the specified location is sought to provide rehabilitative therapy services. The role involves assessing and developing treatment plans for patients in a SNF setting, ensuring quality care in compliance with facility protocols and clinical standards. The position is scheduled for daytime shifts, offering an opportunity to contribute to patient recovery and well-being.
Responsibilities
- Conduct patient assessments
- Develop individualized treatment plans
- Document progress
- Collaborate with the healthcare team to optimize patient outcomes
Schedule
- Days shift, 8 hours per day
- Minimum of 40 hours per week
- Assignment runs from 12/07/2026 through 03/06/2027
- Float rotation may be required
Required Qualifications
- Valid state license as a Physical Therapist
- Graduation from an accredited university or college
- Completion of a state-approved Physical Therapist program
- Current CPR certification
- Applicable state licensure
- At least 0 years of experience in a SNF environment

- Experience working in a Skilled Nursing Facility setting
- Familiarity with facility SOPs
