Responsibilities

• Conduct comprehensive PT evaluations and develop individualized plans of care for residents

• Implement evidence-based interventions to improve mobility, strength, balance, endurance, and functional independence

• Monitor patient progress, document outcomes, and adjust treatment plans as needed

• Collaborate with nursing, nursing assistants, and other therapy staff to coordinate care and optimize outcomes

• Educate residents and families on home exercise programs, safety strategies, and functional goals

• Ensure accurate, timely documentation and maintain compliance with regulatory standards and facility policies

Requirements

• Active Physical Therapist license; Maine licensure or eligibility to obtain Maine licensure

• Doctor of Physical Therapy (DPT) degree or equivalent

• Experience in skilled nursing or geriatric rehabilitation preferred

• Strong assessment, treatment planning, and documentation skills

• Ability to work effectively in a travel assignment and adapt to a new team and environment

• Excellent communication and teamwork abilities
