Travel Physical Therapist Skilled Nursing Facility Job
Responsibilities
- Conduct initial evaluations and develop individualized plans of care for residents in a SNF setting
- Design and implement therapeutic programs to improve mobility, strength, balance, gait, and activities of daily living
- Provide evidence-based interventions including therapeutic exercises, gait training, ROM, manual therapy, and modality application as appropriate
- Re-assess progress regularly, document outcomes, and communicate updates to the interdisciplinary team and care partners
- Monitor safety, ensure proper infection control, and adjust treatment plans to meet patient goals and discharge planning requirements
- Educate residents and caregivers on home programs and strategies to maintain gains after therapy ends
Requirements
- Active Physical Therapist license (Connecticut) or eligibility to practice in the state
- Doctor of Physical Therapy (DPT) degree or equivalent
- Ability to work in a skilled nursing facility setting
- Strong clinical documentation and communication skills
- Commitment to collaborating with an interdisciplinary care team
- Ability to meet assignment-specific credentialing and scheduling requirements

This travel assignment starts on 08/01/2026 for 13 weeks, offering $2,257 to $2,408 per week with guaranteed 35 hours per week. Shift is 5x8 days (08:00–16:30).
