Travel Physical Therapist Skilled Nursing Facility Job
Responsibilities
- Perform initial assessments and develop individualized plans of care for residents requiring physical therapy
- Deliver evidence-based interventions, including therapeutic exercises, gait and balance training, ROM and strength work, and functional mobility activities
- Monitor progress, document outcomes, and adjust treatment plans to support functional goals
- Collaborate with an interdisciplinary team (nursing, therapy colleagues, and case management) to optimize resident outcomes and care planning
- Educate residents and families on home exercise programs, safety, and strategies to maintain functional gains; ensure patient and family comprehension
Requirements
- Active Physical Therapist license in California
- Master’s or Doctorate degree in Physical Therapy
- Experience in geriatric rehabilitation or skilled nursing facility settings preferred
- Ability to work on-site in a skilled nursing facility environment
- Willingness to participate in a travel assignment and adapt to a dynamic care team

This travel assignment starts 07/20/2026 and runs for 13 weeks.
Pay is $2,510 to $2,557 per week, with guaranteed hours of 36 per week.
Shift is 5x8 days, with daily hours from 08:00 to 16:30.
