Travel Physical Therapist Skilled Nursing Facility Job

Responsibilities
- Perform initial evaluations and ongoing reassessments of residents to determine PT needs and goals.
- Develop and implement individualized treatment plans to improve mobility, strength, balance, gait, and functional independence.
- Provide therapeutic exercises, manual therapy, balance training, wheelchair and transfer training, and other evidence-based interventions.
- Document patient progress, modify plans as needed, and communicate findings to the interprofessional care team.
- Support discharge planning and provide patient and caregiver education, including home exercise programs.
- Ensure compliance with regulatory requirements and maintain accurate, timely documentation.
Requirements
- Ability to work in a skilled nursing facility setting with an older adult population.
- Strong communication and collaboration skills to coordinate with the care team.
- Demonstrated clinical judgment, documentation accuracy, and adherence to professional standards.
- Flexibility to accommodate a travel assignment and the outlined shift schedule.
- Willingness to comply with facility policies and procedures.
