Responsibilities
- Conduct comprehensive PT evaluations and develop individualized plans of care for residents
- Deliver evidence-based PT interventions including therapeutic exercises, gait training, balance training, and ADL rehabilitation
- Maintain accurate documentation in the electronic health record and ensure compliance with regulatory standards
- Promote fall prevention, infection control, and safe patient handling during sessions
- Monitor progress, document outcomes, and adjust treatment plans to maximize function and safety
- Collaborate with the interdisciplinary team, communicate with families, and contribute to discharge planning
Requirements
- Active physical therapy license as required by state regulations
- Ability to work in a skilled nursing facility setting; travel assignment experience is a plus
- Experience in SNF or geriatric rehabilitation is preferred
- Strong clinical assessment, documentation, and communication skills; proficient with standard EHR systems
- Eligible to work in the United States

Pay and Schedule
Weekly pay: $2,504–$2,640 per week (per week)
Guaranteed hours: 35 per week
Contract duration: 13 weeks
Shift: 5x8 Days
