Responsibilities

• Evaluate residents’ physical abilities and develop individualized treatment plans in the long-term care setting
• Provide therapeutic interventions including gait training, therapeutic exercises, balance training, ROM, and activity of daily living optimization
• Document assessments, progress, and outcomes; adjust plans based on resident response and goals
• Collaborate with the interdisciplinary team to support discharge planning, caregiver education, and safety initiatives
• Ensure patient safety, apply appropriate modalities, and maintain equipment and infection control standards

Requirements

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
• Active Physical Therapist license in Washington or eligibility for licensure in WA
• Experience in geriatric care or long-term care settings preferred
• Ability to work in a long-term care environment and collaborate with a multidisciplinary team
• Strong documentation, communication, and patient care skills; CPR/BLS preferred
