Travel Physical Therapist Skilled Nursing Facility Job

Responsibilities

• Conduct initial evaluations and ongoing assessments of residents to determine PT goals and progress
• Develop and implement individualized treatment plans focusing on mobility, strength, balance, and functional activities
• Provide patient-centered therapy sessions, including gait training, range-of-motion exercises, strengthening, and pain management as appropriate
• Document progress notes, communicate with the multidisciplinary team, and update the plan of care as residents improve or change
• Monitor safety, maintain infection control standards, and ensure proper use of equipment; contribute to discharge planning and transition planning when appropriate
• Collaborate with nursing staff and other rehabilitation professionals to optimize rehabilitation outcomes and maintain consistent documentation

Requirements

• Doctor of Physical Therapy (DPT) degree or equivalent
• Active Physical Therapist license in the state of New Hampshire
• Basic Life Support (BLS) certification or equivalent at the time of employment
• Minimum 1 year of PT experience in a skilled nursing facility or geriatric rehabilitation setting preferred
• Ability to work a 13-week travel assignment with guaranteed 40 hours per week; eligible to work in the United States
• Strong communication skills and ability to work as part of a multidisciplinary team; comfortable documenting in an electronic health record
