Responsibilities
- Conduct initial and ongoing physical therapy evaluations for residents in a skilled nursing setting and develop individualized treatment plans
- Implement and adjust therapy programs, including therapeutic exercises, gait training, balance and posture activities, ROM and strengthening, and functional mobility tasks
- Perform progress notes, document outcomes, and modify plans of care in collaboration with the interdisciplinary team
- Monitor patient safety, apply appropriate modalities and assistive devices per policy, and ensure adherence to infection control and safety standards
- Communicate clearly with residents, families, and team members regarding goals, progress, and discharge planning
Requirements
- Active physical therapy license in Maryland (or eligibility to obtain)
- A Doctor of Physical Therapy (DPT) or Master of Physical Therapy degree from an accredited program
- Experience with geriatric or skilled nursing facility rehabilitation preferred
- Strong clinical documentation and communication skills; ability to work within an interdisciplinary team
- Ability to meet a 35-hour per week schedule and participate in the assigned travel assignment
- Ability to pass background check and drug screen
