Travel Physical Therapist - Outpatient - $2,545 per week

Physical Therapist (Outpatient) located in the outpatient setting, this role involves providing physical therapy services to patients under the supervision of a licensed healthcare professional. The assignment runs from September 14, 2026 to December 14, 2026 (13 weeks), 40 hours per week on a rotating shift schedule, Monday‑Friday, approximately 8:00 am – 4:30 pm.

Requirements

• Active Illinois license or confirmation that application process has been initiated
• BLS Certification
• Minimum of 1 year of experience
• Permanent address within 50 miles of the facility
• Local rate applies to clinicians with permanent addresses within 50 miles
• OSF Call Off Policy: equivalent of 12 hours every 13 weeks with no penalty or need to pay for the shift
• Orientation hours: 12 hours, non‑billable, guaranteed hours not applying during orientation
• Charting system: Electronic Patient Record (EPR) system: EPIC
• BSN required: No
• Trauma Level: Care Setting: Outpatient; Division: Outpatient
• Requests for time off and last four digits of SSN are part of submission requirements

Schedule
