• Experience: 1 year of previous experience
• Certifications: BLS
• State License: Required
• Driver's License: Required
• Work History/Resume: Required
• Skills Checklist: Must have been completed within 1 year of submission date
• Licence Verification: Required
• Supervisory Reference: 1 required at time of submission
• Taxable Address: Must be outside of 50 miles from facility

Preferred Experience and Skills

• Travel Contract: At least 1 completed
• EMR Experience: Epic preferred
• Copy of Required Certifications: Preferred

• Not an Employee of UnityPoint: In the last 12 months
