About the Role
MetaSense, Inc. is seeking a travel Long Term Care (LTC) Physical Therapist for a travel job in Lowell, Massachusetts.
Requirements
- 1+ year of PT experience – Required
- LTC or Rehab experience – Required
- Active Massachusetts PT license – Required
- New graduates are welcome with Level II clinical experience; Level I observation-only experience does not qualify.
Responsibilities
- Evaluate and rehabilitate patients with physical functioning disorders and/or wound care needs.
- Develop and implement appropriate plans of care following regulatory and clinical practice standards.
- Utilize standardized assessments and evidence-based practices.
- Provide manual techniques, ambulation training, therapeutic exercises, modalities, supportive/assistive devices, wound care, and other appropriate interventions.
- Monitor patient progress and modify treatments based on clinical outcomes.
- Evaluate treatment effectiveness and make referrals when appropriate.
- Provide education, consultation, and counseling to patients, families, caregivers, and other healthcare providers.
- Communicate effectively with the healthcare team regarding patient progress, barriers, and treatment plans.
- Complete comprehensive discharge summaries and maintain accurate electronic documentation.
- Accurately record treatment minutes in accordance with regulatory requirements.
- Participate in patient care conferences, utilization review meetings, rehabilitation meetings, trainings, in-services, and other required activities.
- Obtain necessary DME and supplies as needed.
- Supervise and support Physical Therapist Assistants, rehabilitation aides, and/or students.
- Practice in accordance with APTA standards and Massachusetts state regulations, ethics, and professional requirements.
