Responsibilities
- Providing rehabilitation services to help patients regain mobility and independence
- Assessing patient conditions and developing personalized treatment plans
- Documenting patient progress accurately and efficiently
- Deliver long-term care rehabilitation, evaluate patient needs, collaborate with healthcare team, and ensure compliance with licensing standards.
Qualifications
- Graduation from an accredited physical therapy program
- Current Indiana PT license (or applicable state license)
- CPR/BLS certification
- Experience in long-term care or rehabilitation settings is preferred
- Strong assessment, communication, and documentation skills
