Travel Physical Therapist Home Health - $2,820 per week

The Physical Therapist Home Health (PT HH) position is based in Medford, Wisconsin. Job Details

Location: Medford, Wisconsin. The schedule is days, Monday through Friday, 8:00 AM to 5:30 PM, with 40 hours per week guaranteed. The contract length is 13 weeks with the possibility of extension, starting on August 24, 2026.

Requirements

• Master's Degree in Physical Therapy
• Wisconsin state licensure
• American Heart Association (AHA) or ARC Basic Life Support (BLS) certification
• National Provider Identifier (NPI)
• Prior home health experience, preferably working with adult/geriatric patients with orthopedic, neurological, and mobility diagnoses
• At least one year of experience is preferred

Additional Information

Occasional travel to central and northern regions may be required, with mileage reimbursed at the government-approved rate. The Saturday rotation involves working one Saturday every 6-8 weeks, with a weekday rotation off. No holidays are included. During flu season, unvaccinated travelers are required to wear a mask at all times per facility policy. Scrubs or business casual attire are acceptable. The contract includes 36 hours of low census unbillable hours.

Pay

The pay package is based on 8 hour shifts and 40 hours per week, with a tax-free stipend amount to be determined.
