Travel Physical Therapist - Home Health - $2,815 per week

Care Team Solutions is seeking a travel Home Health Physical Therapist for a 13-week assignment starting July 27, 2026, in Gloucester, Virginia.

Responsibilities

• Provide physical therapy services to patients in their homes.
• Develop and implement individualized treatment plans based on patient needs.
• Assess patient progress and adjust treatment plans accordingly.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
• Document all patient interactions and treatments accurately.

Requirements

• Current licensure as a Physical Therapist in Virginia.
• At least 2+ years of recent rehabilitation experience.
• Knowledge of home health care protocols and standards.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.

Qualifications

• Master's degree in Physical Therapy preferred.
• Pass a background check and drug screening.
• Valid driver’s license and reliable transportation.

Pay
