The role involves delivering specialized therapy interventions to enhance patient recovery and independence, with an emphasis on timely documentation and communication. Job Details Position: Physical Therapist (Home Health) Specialty: Home Health Location: Up to a 60-mile radius from the hub/office; locals accepted and at travel rate Shift: Days, 8 hours per shift, with a minimum of 40 hours per week Certifications and Education: Current BLS certification, active drivers license in hand, and proficiency in HCHB (Homecare Homebase) are required Experience: Minimum of 1 year of relevant experience in home health, OASIS experience required for staff completing Start of Care (SOC); recent experience preferred Additional Requirements: Timely communication and documentation are mandatory; RTO requests must be submitted at the time of application; on-call rotation may be required Responsibilities: Providing comprehensive physical therapy assessments and interventions, utilizing interdisciplinary resources, and supporting patients with a variety of diagnoses and post-operative conditions Submission Requirements: All candidates must have an active drivers license, transport themselves independently, and have completed Monday orientation. Former employees must be separated for at least 6 months before reapplying as travelers.
