Travel Physical Therapist - Home Health - $2,526 per week

Description

American Traveler is seeking a licensed Physical Therapist with at least 1 year of home health experience for a 13-week travel contract serving patients across the Hawaiian Islands, including the Big Island, Kauai, Maui, and Oahu.

Requirements

  • Minimum 1 year of home health experience required
  • OASIS documentation experience of more than 3 months required
  • Current BLS certification (AHA) required
  • Active Hawaii (HI) PT license required; license required if applicable
  • Strong skills in geriatric, neurologic, and orthopedic patient populations
  • Proficiency with adaptive and assistive devices, including Hoyer lifts, gait belts, walkers, sliding boards, wheelchairs, lower extremity prosthetics, and supplemental oxygen
  • Experience with OASIS documentation, plan of care development, reassessments, and discharge planning
  • Ability to identify environmental barriers and conduct home safety assessments

About the Role

Home health setting serving a predominantly geriatric patient population (kupuna) in their residences across Hawai'i Island

Responsibilities

  • Occasional floating to cover Kauai, Maui, and/or Oahu may be required; all float-related expenses are covered
  • Patient specialties include orthopedics, neurology, geriatrics, cardiopulmonary conditions (COPD, CHF, CABG, MI, PVD), post-stroke rehabilitation, and cognitive/neurological disorders such as Parkinson's disease and dementia
  • Procedures and skills include gait training, OASIS assessments, plan of care development, patient/family education, home exercise program development, and post-op rehabilitation (total hip/knee replacement, spinal surgery)
  • Productivity requirement of 25 units per week
  • 1:1 patient ratio
  • Day shift schedule, 5x8 hours (8:00–16:30)
  • HomeCare HomeBase (HCHB) used for documentation; experience preferred
